Surgical decompression of a tension pneumomediastinum. A ventilatory complication of status asthmaticus.

چکیده
A case of status asthmaticus was associated with cardiorespiratory arrest, illustrating rarely reported complications of ventilatory therapy including tension pneumomediastinum and coronary air embolization. Proposed pathophysiologic mechanisms and recommendations for surgical management are discussed.

Pneumomediastinum and subcutaneous emphysema in status asthmaticus, requiring surgical decompression.
Leakage of air into the interstitial tissues of the lungs of sufficient degree to produce pneumomediastinum and subcutaneous emphysema is an uncommon and potentially fatal complication of bronchial asthma in childhood. Another possibly fatal complication of similar causation is tension pneumothorax. It is possible that interstitial emphysema occurs more frequently in severe asthma and allied st...
متن کاملEmergency percutaneous needle decompression for tension pneumoperitoneum
BACKGROUND Tension pneumoperitoneum as a complication of iatrogenic bowel perforation during endoscopy is a dramatic condition in which intraperitoneal air under pressure causes hemodynamic and ventilatory compromise. Like tension pneumothorax, urgent intervention is required. Immediate surgical decompression though is not always possible due to the limitations of the preclinical management and...
متن کاملPressure-controlled ventilation in children with severe status asthmaticus.
OBJECTIVE The optimum strategy for mechanical ventilation in a child with status asthmaticus is not established. Volume-controlled ventilation continues to be the traditional approach in such children. Pressure-controlled ventilation may be theoretically more advantageous in allowing for more uniform ventilation. We describe our experience with pressure-controlled ventilation in children with s...
متن کاملSubcutaneous emphysema, pneumomediastinum, and potentially life-threatening tension pneumothorax. Pulmonary complications from arthroscopic shoulder decompression.
Subcutaneous emphysema, pneumomediastinum, and tension pneumothorax are previously unreported complications of shoulder arthroscopy with subacromial decompression. Three patients developed extensive subcutaneous emphysema, pneumomediastinum, and bilateral tension pneumothorax during or immediately after shoulder arthroscopy with subacromial decompression. The procedure was terminated and approp...
متن کاملTension Pneumoperitoneum: A Rare Complication of Upper Gastrointestinal Endoscopy
INTRODUCTION Tension pneumoperitoneum is defined as the massive accumulation of air in the peritoneal cavity, which results in a sudden increase in intraabdominal pressure. Various iatrogenic procedures are responsible for this complication. We herein report a case of tension pneumoperitoneum resulting from upper gastrointestinal endoscopy. CASE DESCRIPTION A 45-y-old man was referred to our...
